Augusta, Inc. - In the Oxide plant, an employee was provided and required to use a full face negative pressure respirator for protection against lead exposure. An annual fit test for the full face was not performed, on or about 10/30/19.    b) U.S. Battery Mfg. Augusta, Inc. - In the Oxide plant, an employee was provided and required to use a half mask negative pressure respirator for protection against lead exposure. The last fit test was performed on 8/28/18 and not repeated within 12 months, on or about 10/30/19.

29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead:    a) U.S. Battery Mfg. Augusta, Inc. - At the Oxide plant break room, employees were exposed to ingestion hazards from lead contamination found on the refrigerator handle. Lead contamination measured was 272.7 micrograms, on or about 10/31/19.    b) U.S. Battery Mfg. Augusta, Inc. - At the Oxide plant break room, employees were exposed to ingestion hazards from lead contamination found on the microwave control pad. Lead contamination measured was 14.5 micrograms, on or about 10/31/19.    U.S. Battery Mfg. Augusta, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1025(h)(1), which was contained in OSHA inspection number 1353892, citation number 1, item number 7 and was affirmed as a final order on 3/15/19, with respect to a workplace located at 1895 Tobacco Rd, Augusta, GA.

Augusta, Inc. - In the Oxide plant, an employee was removed from the site due to a blood lead level above 60 ug/dL. An incident report OSHA 301 or equivalent form was not completed. The information documented did not state what the employee was doing before the exposure that caused the increased of lead in blood and how the incident occurred, on or about 10/30/19.
