29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) At the jobsite, on or about November 5, 2019, employees used a Werner aluminum extension ladder that did not extend 3 feet above the upper landing surface to access and exit the second working level of a new apartment building during wood framing activities, resulting in fall hazards of up to 11.2 feet onto earthen ground.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) At the jobsite, on or about November 5, 2019, employees working on upper levels while performing wood framing activities were exposed to fall hazards of more than 21 feet and fall protection was not provided.    Vastu Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1192278, citation number 2, item number 1, and was affirmed as a final order on April 11, 2017, with respect to a workplace located at 7 North Main Street, Old Saybrook, CT 06475.    Vastu Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1142142, citation number 1, item number 1a, and was affirmed as a final order on June 28, 2016, with respect to a workplace located at 7 North Main Street, Old Saybrook, CT 06475.
