29 CFR 1910.178(m)(3): A safe place to ride was not provided where riding a powered industrial truck(s) was authorized:       a. At the outside of the building, nearest the facilitys main entrance doors, located at 8955 Beach Blvd, Jacksonville, FL 32216: On or about December 9, 2019, and at times prior, the employer exposed an employee to fall hazards, in that an employee was installing Christmas lights standing on the unprotected side or edge of a shelving rack that was lifted and supported by the forks of a forklift .       OR IN THE ALTENATIVE: 29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:

At Number One Vending Services, Inc., located at 8955 Beach Blvd, Jacksonville, FL 32216: On or about December 9, 2019, and at times prior, the employer did not ensure each powered industrial truck operator was competent to operate a powered industrial truck safely, in that employer did not provide employees powered industrial truck training and evaluation.
