29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) Creative World School, Vinings, GA: Trench: On or about 12/16/2019, and at times prior, the employer exposed employees to struck-by hazards while working inside a 6-feet, 3-inch deep trench/excavation, in that head protection was not provided to employees engaged in reinforced concrete pipe (RCP) sewer installation inside the trench.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) Creative World School, Vinings, GA: Trench: On or about 12/16/2019, and at times prior, the employer exposed employees to entrapment hazards while working in an excavation which was more than 6-feet deep and 23-feet long, in that a safe means of egress was not provided while performing sewer line work inside the trench.
