29 CFR 1917.45(i)(5)(i)(C): A minimum clearance of 4 feet was not maintained between energized power lines and cranes in transit with no load and the boom lowered. Champion Auto Ferry Inc., Mainland terminal, on or about December 11, 2019, the employer did not ensure that Grove Hydraulic Crane Model # 58d, S/N # 79057 boom was lowered prior to backing up the crane to park it. The boom struck energized power lines which resulted potential electrocution hazards.

Champion Auto Ferry Inc., Mainland terminal, On or about December 11, 2019, the employer did not ensure that protective helmets were worn while an employee was working below the Grove Hydraulic Crane Model # 58d, S/N # 79057 boom connecting metal grating to be lifted from the water's edge due to a collapsed dock. The employee operating in this condition was exposed to struck by hazard(s).

Champion Auto Ferry Inc, Mainland terminal, On or about December 11, 2019, the employer did not implement an effective rigging inspection program that included a quarterly inspection of a two leg chain sling utilized to remove metal grating that had fallen into the water due to an engineering failure of an auto ferry that struck the pier.
