Mackinac Island Ferry Company, at a worksite located at 587 North State St., Saint Ignace, MI 49781: On or about April 10, 2024, the employer failed to ensure that the OSHA Form 300 for calendar year 2024 contained a complete description in column F. The 2024 300 log did not define the object that directly caused the injury to the employee.
