29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    A.)  Alex Roofing Company, Inc., 3901 Pacific Avenue, Erie, PA  16506, west side of the structure, canopy roof, on or about December 27, 2019 and times prior thereto:  Employees were performing roofing activities that included, but were not limited to, installing the cap on the peak of the roof structure.  Employees were not utilizing means or methods of fall protection when exposed to an eave-to-ground height fall hazard of 11.7-feet, and a peak-to-ground height fall hazard of 18.2-feet.    ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED:  Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  A.)  Alex Roofing Company, Inc., 3901 Pacific Avenue, Erie, PA  16506, south side of the structure, main roof, on or about December 27, 2019 and times prior thereto:  Employees were performing roofing activities that included, but were not limited to, installing the cap on the peak of the roof structure.  Employees were not utilizing means or methods of fall protection when exposed to an eave-to-ground height fall hazard of 11.7-feet, and a peak-to-ground height fall hazard of 23.3-feet.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
