29 CFR 1910.134(m)(2):  The employer did not establish an adequate record of the qualitative or quantitative fit test administered to employees:      a)  In the Facility, on or about January 7, 2020 - The employer did not establish an adequate record of the fit testing provided to employees.      ABATEMENT NOTE:      The fit test record administered to employees shall include:    a)  The name or identification of the employee tested;    b)  The type of fit test performed;    c)  The specific make, model, style and size of the respirator tested;    d)  The date of the test;  and    e)  The pass/fail results for qualitative fit tests or the fit factor and strip chart recording or other recording of the test results for quantitative fit tests.
