29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems or personal fall arrest systems: a) On or about January 17, 2020, on the west side of the house; employees were observed on a roof with a 7 x 12 pitch without the use of fall protection systems at a height of 16 feet.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder: a) On or about January 17, 2020 on the west side of the house; Employees utilized a Werner extension ladder to access a roof of a house that did not meet the ladder position with the horizontal distance greater than the one-quarter of the working length height.
