29 CFR 1917.112(b)(1): Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet (1.22 m) or into the water, except as specified in paragraph (b)(2) of this section:    a) On or about January 22, 2020 on the walkway leading to the dock area employees were exposed to fall hazards of approximately 5 foot, 6 inches to 7 foot, 2 inches and the employer did not ensure guardrails were provided at locations where employees were exposed to floor openings.

29 CFR 1910.305(g)(1)(iv)(A): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables were used as a substitute for the fixed wiring of a structure:  a) On or about January 22, 2020 in the loading cell house a flexible cord set was used to power a relocatable power tap and associated equipment exposing employees to electrical hazards and the employer did not ensure flexible cords were not used as a substitute for the fixed wiring of a structure.
