On or about January 24, 2020, in the warehouse area, the employer did not ensure the two tier industrial storage racks were properly anchored to the floor.   b.  On or about January 24, 2020, in the warehouse area, the two tier industrial storage racks were overloaded, per the posted rated load.

On or about January 24, 2020, in the warehouse area, the extension cord used to supply power to a floor scale did not have a strain relief connecting the cord to the plug and the insulated wires inside the cord were visible.
