29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  a) New residential construction located on Lot #19, Roof and second floor: On or about 01/28/2020, and at times prior, the employer exposed employees to struck-by hazards while operating circular saws and pneumatic nail guns, in that protective eyewear was not provided to employees operating these tools while engaged in framing activities.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) New residential construction located on Lot #19, Roof: On or about 01/28/2020, and at times prior, the employer exposed employees to falls of 18-feet to the lower level, in that a fall protection system was not used while engaged in sheathing activities on a 12/12 pitched roof.   b) New residential construction located on Lot #19, Second level: On or about 01/28/2020, and at times prior, the employer exposed employees to falls of 11-feet, 4-inches to the lower concrete surface below, in that a fall protection system was not provided on the second level of the structure while employees were engaged in framing activities.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) New residential construction located on Lot #19, Second floor job-made scaffold: On or about 01/28/2020, and at times prior, the employer exposed employees to falls of 19-feet, 7-inches to the lower level, in that a fall protection system was not used while engaged in framing activities on a job made scaffold platform.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  a) New residential construction located on Lot #19, Roof and second floor: On or about 01/28/2020, and at times prior, the employer failed to inspect all of the electrical cords prior to the start of the shift. A 50-ft. flexible cord used to power a Makita (Model 5007F) 7-1/4-inch circular saw had a torn outer casing, and a damaged strain relief and was not removed from service.
