29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:      a) On February 4, 2020, employees engaged in carpentry work were not protected from fall hazards while working on the roof ledge with the ground to eave height of approximately 17 feet.     b) On February 4, 2020, employees engaged in carpentry work were not protected from fall hazards of approximately 10 feet to the ground while working near an unprotected floor opening.      c)  On February 4, 2020, employees engaged in carpentry work were not protected from fall hazards of approximately 10 feet to the ground while working near an unprotected wall opening.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1052(c)(1)(ii):  Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one stairrail system along each unprotected side or edge:      a)  On February 4, 2020, employees were exposed to the hazard of falling while using stairways that were not equipped with a stairrail system.     No abatement certification or documentation is required for this item.

29 CFR 1926.503(b)(1):  The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:    a)  On or about February 4, 2020, the employer did not maintain records or certification of fall protection training for employees working onsite.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
