29 CFR 1910.147(c)(1): The employer did not establish a program consisted of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source, and rendered inoperative:  A lockout/tagout program, including written procedures for specific machines or operations, such as when working on electrical equipment, training of employees who perform lockout/tagout, providing lockout devices and locks, and periodic inspections, was not established and implemented for employees performing work requiring lockout, such as, but not limited to, working on electrical equipment, such as changing and installing circuit breakers in live electrical panels.  Abatement documentation is required for this violation.

At the circuit breaker panel next to the produce prep area, there was no lighting over the panel.  Employees used circuit breakers to turn on/off store lighting.    b. In the motor control rooms, there was no lighting, including for changing out circuit breakers in electrical panel boxes not able to be de-energized.  Abatement documentation is required for this violation.
