Located at 1795 Franklin Park South, Columbus, Ohio:  On or about February 04, 2020, the employer did not initiate and maintain a safety and health to ensure hazard detection and correction, while employees performed remodeling tasks at the worksite.  Employees were exposed trip hazards from the unprotected holes on the floor and electric shock from exposed live parts.

29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  a.  Located at 1795 Franklin Park South, Columbus, Ohio:  On or about February 04, 2020, the employer failed to instruct employees on the avoidance of unsafe conditions while they performed remodeling work at at a residential worksite.   Employees were exposed to trip hazards from unprotected holes in the floor and electric shock from exposed live electrical parts.

29 CFR 1926.403(i)(2)(i): Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means:  a.  Located at 1795 Franklin Park South, Columbus, Ohio:  On or about February 04, 2020, the employer failed to ensure that live parts from the unguarded electrical box was protected from accidental contact.  The exposed live parts were not protected in an enclosed panel board cabinet.  The employees were therefore exposed to potential electric shock from exposed live parts.  b.  Located at 1795 Franklin Park South, Columbus, Ohio:  On or about February 04, 2020, the employer failed to ensure that live parts from the unguarded electrical outlet wiring in the basement, was protected from accidental contact.  The electrical outlet did not have face plate to encase the live parts.  The employees were therefore exposed to potential electric shock from exposed live parts.  c.  Located at 1795 Franklin Park South, Columbus, Ohio:  On or about February 04, 2020, the employer failed to ensure that live parts from the unguarded electrical wiring at the stairway to the upper level was protected from accidental contact.  The electrical wiring did not have face plate to enclose the wiring.  The employees were therefore exposed to potential electric shock from exposed live parts.  d.  Located at 1795 Franklin Park South, Columbus, Ohio:  On or about February 04, 2020, the employer failed to ensure that live parts from the unguarded electrical outlet and box at the main floor level, was protected from accidental contact.  The electrical outlet did not have a face plate to enclose the live parts.  The employees were therefore exposed to potential electric shock from exposed live parts.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers.  a.  Located at 1795 Franklin Park South, Columbus, Ohio:  On or about February 04, 2020, the employer failed to ensure that the holes on the working surface at the main floor level, were protected, securely covered and marked, while employees conducted remodeling work at the main floor level.  The employees were exposed to fall and trip hazards as a result.   b.  Located at 1795 Franklin Park South, Columbus, Ohio:  On or about February 04, 2020, the employer failed to ensure that the holes on the working surface at the third floor level were protected, securely covered and marked, while employees conducted remodeling work.  A ceramic tile was placed over the floor hole opening, measured approximately 28 inches long and 4 inches wide, that was not secured to the working surface. The employees were exposed to fall and trip hazards as a result.   c.  Located at 1795 Franklin Park South, Columbus, Ohio:  On or about February 04, 2020, the employer failed to ensure that the holes on the working surface at the third floor level was protected, securely covered and marked, while employees conducted remodeling work.  The employer did not cover the floor opening, measured approximately 12 inches long and 9 inches wide.  The employees were exposed to fall and trip hazards as a result.
