29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition: (a) On or about 02/13/2020 - File room, ceiling tiles and carpet were not kept in a sanitary condition in that discoloration including, but not limited to, water stains was observed. (b) On or about 02/13/2020- Main office space, ceiling tiles and carpet were not kept in a sanitary condition in that discoloration including, but not limited to, water stains was observed. (c) On or about 02/13/2020 - Personal office space, ceiling tiles were not kept in a sanitary condition in that discoloration including, but not limited to, water stains was observed. (d) On or about 02/13/2020 - Facility, water leaks were observed in the file room and the room near the entryway and ceiling tiles were sagging in the hallway, file room, and some offices from water intrusion.
