Bags, containers, bundles, etc., was stored in tiers that were not stacked, blocked, interlocked and limited in height so that they were stable and secure against sliding or collapse:   Yard Area:  Piles of shredded rubber were stacked in piles greater than twenty (20) feet without protection to prevent collapse or sliding of the material into the employee work area exposing them to struck-by hazards.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushed-by hazards from moving vehicles and heavy equipment:  a) Yard:  Employees were exposed to struck-by and crushed-by hazards while walking and working in an area where dump trucks, tractor trailers, wheel loaders, and other heavy equipment was being operated without a traffic control plan;  b) Yard:  Employees were exposed to struck-by hazards where wheel loaders were used with snowplow attachments that were not authorized by the manufacturer for pushing used tires around the yard where employees were walking and working.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Used tire storage and painting area:  Employees engaged in receiving used tires from the ground via a conveyor are exposed to caught-in hazards from an in-going nip point where the conveyor belt had a gap of greater than three (3) inches between the roller and wall.
