29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      a) Bottle room - On or about February 21, 2020 and at times prior thereto, employees are exposed to ingoing nip points and rotating parts while cleaning behind and/or unjamming the bottle machines.  The back panels of the machines are regularly left off and/or not secured to the machine.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:   a) Back room - On or about February 21, 2020, an employee was hospitalized for an amputation following an incident on 2-21-20 approximately 7:15 pm.  The incident was reported three days after by the NYS Police on 2-24-20.
