29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a.  On or about September 18, 2019, in the parking lot area located at 3128 Tampa Rd, Oldsmar, FL, two employees in the basket of a JLG 800 AJ aerial lift, were not wearing a body belt, as the aerial lift was being operated-driving through the parking lot, exposing the employees to an ejection hazard.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold (including aerial lifts) trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a.  On or about September 18, 2019, in the parking lot area located at 3128 Tampa Rd, Oldsmar, FL, an employee who had not been trained on the safe operating procedures for an aerial lift was operating a JLG 800 AJ aerial lift.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  a.  On or about September 18, 2019, in the parking lot area located at 3128 Tampa Rd, Oldsmar, FL, two untrained employees in the basket of an aerial lift, were not wearing a body belt or other forms of fall protection, as the aerial lift was being operated-driving through the parking lot, exposing the employees to an ejection and fall hazards.
