29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about March 10, 2020, the employer failed to ensure that the horizontal bandsaw had machine guarding methods in place to protect employees. The barrier guard on the top of the saw blade had six (6) openings about eight and 1/4 inches (8.25") wide and about thirty inches (30") in length. These openings on the guard were about thirteen inches (13") from the rotating blade.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about February 24, 2020, the employer failed to establish a lockout/tagout program when employees are tasked with changing the blade on the horizontal bandsaw. Energy sources included, but not limited to electrical, hydraulic, and/or pneumatic.
