29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:  Employees were exposed to the following struck-by hazards in the following locations:  a) Steel Yard (South Side)      1. Rack #1: Steel Slip track loose and unsecured on top of  the rack, approximately 15' from the ground.     2. Rack #2: H Steel loose and unsecured on top of the rack, approximately 15' from the ground.     3. Rack #3: Steel furring loose and unsecured on top of the rack, approximately 15' from the ground.   b) Steel Yard (East Side)      1. Rack #4: Steel Resilient Channel/High Hat loose and unsecured on top of the rack, approximately 15' from the ground.     2. Rack #5: Steel Resilient Channel/High Hat loose and unsecured on top of the rack, approximately 15' from the ground.  c) Lumber Yard (Metal Conex boxes - gap between Box 3 and 4, and on top of Box 5, Box 6, Box 7 and Box 9)      1. Wooden Pallets, mold stock and PVC Pipes were unsecured and loose on top of the boxes, approximately 10' from the ground.  d) Lumber Yard (Insulation Area)      1. Wooden support column holding wooden shelf containing batts of insulation, approximately 10' from the ground was damaged and leaning.

29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards and general slip or trip hazard.  Location: Lumber Yard/Insulation Area - Conex Box 4 and Conex Box 5.  Employees were exposed to tripping hazards while walking into Conex Box 4 and Conex Box 5 with rotting wooden floors while loading and unloading batts of insulation.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s)  Location: Paint Mixing Room  Employees were exposed to a rear unguarded area of a Vortex 2 Paint Mixer while the mixers' frame lowers the mixers into five gallon buckets of paint creating a pinch point between a fixed horizontal metal support bar and the mixer's frame.
