29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Commercial buildings (Suites 100 & 200) located at 4385 Kimball Bridge Road, Johns Creek, GA: Roof: On or about 04/24/2020 the employer exposed employees to fall hazards of 13-feet, 6-inches to the lower level, in that employees engaged in roof tear off and underlayment installation activities on a 10/12 pitched roof were not provided with a fall protection system.

29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators.                                                                   a) Commercial Building (Suite 200) located at 4385 Kimball Bridge Road, Johns Creek, GA: Front side: On or about 04/24/2020, the employer allowed employees to use the material hoist as a ladder to access the roof exposing them to fall hazards of approximately 3-feet, in that the manufacturer states in the general safety instructions not to climb the LH-Series hoist or use as a personnel ladder.
