29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) New residential construction located on (Lot #127) 4811 Electric Avenue, SW, Mableton, GA 30126, East side of the structure: On or about 06/01/2020, the employer exposed employees to struck-by injuries from 1/2-inch plywood sheets that were hoisted by hand to the roof, in that head protection was not provided to employees working at ground level.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  a) New residential construction located on (Lot #127) 4811 Electric Avenue, SW, Mableton, GA 30126, Roof: On or about 06/01/2020, the employer exposed employees to struck-by hazards while operating pneumatic nail guns at 120 PSI, in that protective eyewear was not provided to employees engaged in plywood sheathing installation activities.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) New residential construction located on (Lot #127) 4811 Electric Avenue, SW, Mableton, GA 30126, Roof: On or about 06/01/2020, the employer exposed employees to fall hazards of 10-feet to the lower level, in that a fall protection system was not provided to em while operating pneumatic nail guns at 120 PSI, in that protective eyewear was not provided to employees engaged in plywood sheathing installation activities.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  a) New residential construction located on (Lot #127) 4811 Electric Avenue, SW, Mableton, GA 30126, Front side of the structure: On or about 06/01/2020, the employer failed to inspect electrical cords prior to the start of the shift, in that 50-ft. extension cords used to power circular saws and other equipment had torn and frayed casings and damaged strain relief on both ends of the cords.
