29 CFR  1910.421(d): Assessments made during planning of dive operations did not include all safety and health aspects specified in 29 CFR 1910.421(d)(1) through 29 CFR 1910.421(d)(6):    On or about 06/06/2020, at 8994 Palm Tree Lane, Pembroke Pines, Florida,  the employer did not assess the hazards of the underwater conditions such as but not limited to planning and assessment of safety and health was not performed for the diving mode such as a hookah rig was used (1910.421(d)(1)); underwater conditions and hazards consisting of poor visibility with entanglement hazards (1910.421(d)(2)); the breathing gas supply did not include reserves (1910.421(d)(3); the diving equipment and system lacked a tether and harness to retrieve the diver (1910.421(d)(5); the employer did not provide and assess dive team assignments and physical fitness of the dive team members 1910.421(d)(6).
