29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  At a new commercial construction site located at Creekside Drive, Parkville, Missouri 64152, an employee was exposed to serious struck-by and head injuries. The employee was observed working underneath another employee without adequate head protection.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  At a new commercial construction site located at Creekside Drive, Parkville, Missouri 64152, employees were exposed to serious eye and face injuries. Employees utilizing pneumatic nail guns were not protected from flying particles by using appropriate eye or face protection.

29 CFR 1926.150(a)(2): Access to all available firefighting equipment was not maintained at all times:  At a new commercial construction site located at Creekside Drive, Parkville, Missouri 64152, employees were exposed to serious fire and explosion hazards. A gasoline powered air compressor was operating in close proximity to an open gasoline container and other combustible material. Employees were not protected from fire or explosion hazards by the use of fire extinguishers, which were not accessible to employees on the site.

29 CFR 1926.151(a)(2): Internal combustion engine powered equipment were not so located that the exhausts were well away from combustible materials. When the exhausts are piped to outside the building under construction, a clearance of at least 6 inches shall be maintained between such piping and combustible material.  At a new commercial site construction located at Creekside Drive, Parkville, Missouri 64152, employees were exposed to serious fire and explosion hazards. A Rolair Systems air compressor powered by an internal combustion engine was operating in the bed of a work truck. An open container of gasoline and other combustible materials were in close proximity to the air compressor.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a new commercial construction site located at Creekside Drive, Parkville, Missouri 64152, employees were exposed to serious fall hazards. Employees engaged in roofing work were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Employees were exposed to fall hazards between 9 feet 9 inches and 25 feet to the ground below.

Portable ladders that the employees were utilizing to access the roof did not have their side rails extended at least 3 feet (.9 m) above the upper landing surface being accessed.
