29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  At residential worksites located at 3408 & 3493 NE 85th Ter, KC MO -- employees engaged in roofing duties were using pneumatic nail guns and other various small hand tools were not wearing any eye protection. This exposed the employees to getting debris in their eyes causing injury. This was documented on or about 4/24/2024.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At residential worksites located at 3408 & 3493 NE 85th Ter, KC MO -- employees engaged in roofing duties were not using any kind of personal fall arrest system. This exposed the employees to fall hazards ranging from 9' 6" to 17' 8" ground to eave. This was documented on or about 4/24/2024.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At worksites located at 3408 & 3493 NE 85th Ter KC MO -- employees at both sites across the street from each other were using portable extension ladders to gain access to the roofs. The ladders were not extended 3 feet above the landing surfaces. This exposed the employees to fall hazards ranging from 9' 6" at one and 17' at the other ground to eave and peak. These were documented on or about 4/24/2024.
