29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: (a) Doyle Construction, Inc. at 450 15th Street in Georgetown, CO 80444: On and before July 22, 2020, the employer did not ensure that a competent person conducted daily inspections of a 5-6 foot deep trench with vertical walls, Type B soil, water intrusion, and vibration from nearby road traffic. This condition exposed workers to excavation hazards.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). (a) Doyle Construction, Inc. at 450 15th Street in Georgetown, CO 80444: On and before July 22, 2020, the employer did not ensure that a 5-6 foot deep trench was protected from cave-ins through us of trench box or other shielding system that meets the requirements of 29 CFR 1926.652 (c). This condition exposed workers to excavation hazards.
