29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:   On or about August 20, 2020 at the facility where employees were exposed to electrical hazards when using a Ricoma Ikonix fabric cutter where the power cord had pulled away from the housing.

On or about August 20, 2020 at the facility, where an employee received lacerations to the left hand and was hospitalized without the employer notifying OSHA within 24 hours.
