29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about October 7, 2020, at the above addressed jobsite, an employee using a leaf blower did not wear safety glasses, thereby exposing the employee and the employees around him to the hazards associated with eye injuries

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about October 19, 2020, at the above addressed jobsite, employees performing residential roofing operations, eighteen (18) feet nine (9) inches above the ground were not protected from falls, thereby exposing the employees to fall hazards.  b.  On or about October 19, 2020, at the above addressed jobsite, employees performing residential roofing operations, ten (10) feet four (4) inches above the ground were not protected from falls, thereby exposing the employees to fall hazards.
