Section (5)(a)(1) of the Occupational Safety and Health Act:   The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were not protected from struck-by and crushing hazards from inadequately secured racking system.   a) 1110 W Rezenof Dr, Maintenance Shop:  On or about October 22, 2020, employees performed vehicle and equipment maintenance working under and next to industrial steel storage racks.  Interior and rear column base plates on the frames of the racking system were not anchored to the floor to resist all applicable forces, exposing employees to struck-by and crushing hazards.

29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) Northeast Bay Door, Center Rack:  On or about October 22, 2020, lighting fixtures were installed with a flexible extension cord permanently attached to the rack system near the tire maintenance station.    The relocatable power tap was observed used as a substitute for fixed wiring and attached to the rack with zip ties, exposing employees to electrical and fire hazards.   b) Northwest Bay Door, Center Rack - On or about October 22, 2020, employees used the bottom shelf as a work table to read and write documents, to place hand tools, chemicals, and vehicle parts while performing vehicle maintenance.  A relocatable power tap used as a power source for lighting the area, was connected in series to an extension cord and was not directly plugged into the permanently installed outlet, exposing employees to electrical and fire hazards.  c) Northwest Bay Door, First Rack, bottom shelf:  On or about October 22, 2020, lighting fixtures were installed with a flexible extension cord permanently attached to the rack system when performing vehicle maintenance.  A relocatable power tap was connected in series to an extension cord to provide power for lighting, exposing employees to electrical and fire hazards.

29 CFR  1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv):   a)  Northeast Bay Door, Center Rack:   On or about October 22, 2020, lighting fixtures were powered with a flexible extension cord permanently installed to the rack system near the tire maintenance station.  The extension cord was spliced and hardwired into the light fixture housing and attached to the rack with tape and staples, exposing employees to fire and electrical hazards.   b)  Northwest Bay Door, bottom of Center Rack:  On or about October 22, 2020, lighting fixtures were powered with a flexible extension cord permanently installed to the rack system near the tire maintenance station.  The cord was observed running through openings in racking, on the floor, and through stored items on floor.  This condition subjected the extension cord to damage and exposed employees to fire and electrical hazards.   c) Northwest Bay Door, First Rack, bottom shelf:   On or about October 22, 2020, lighting fixtures were powered with a flexible extension cord permanently installed to the rack system and used when performing vehicle maintenance.  The cord was observed running through openings in racking which subjected the cord to damage, exposing employees to fire and electrical hazards.
