29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Bui Residence, Gate 6 1/2, Sausalito:  Employees were observed installing floor joists and substructure sheeting on the edge of an unprotected work surface without the use of a fall protection system.  Employees were exposed to fall hazards in excess of 10 feet.

29 CFR  1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   Bui Residence,  Gate 6 1/2, Sausalito:   Employees were observed working on an unguarded edge that measured in excess of 10 feet from the working surface to ground and using a ladder that did not extend 3 feet past the working surface.  Employees are exposed to fall hazards and have not received the required training.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Bui Residence, 23 Gate 6 1/2, Sausalito:  Employees were exposed to fall of 8.5 feet when accessing and egressing the main deck of the project.  The ladder did not extend at least 3 feet above the landing surface.

Travel distance from any point of the protected area to the nearest fire extinguisher shall not exceed 100 feet.
