Mechanic Shed - On or about November 3, 2020, an energy control procedure was not developed that addressed the control of potentially hazardous energy when repairs were conducted on the hydraulic system of a Premium Supply Power Hoist System (model: PH 630) that was attached to the bed of a 2008 Chevy C7500 Truck, thus exposing an employee to a crushing hazard.

Mechanic Shed - On or about November 3, 2020, an employee was not provided training that addressed the applicable energy sources, and the methods and means necessary for energy isolation and control when engaged in the repair of the hydraulic system of a Premium Supply Power Hoist System (model: PH 630) that was attached to the bed of a 2008 Chevy C7500 Truck, thus exposing an employee to a crushing hazard.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.  a). Mechanic Shed - On or about and November 3, 2020, the employer did not provide chemical hazard training and information to an employee that utilized hazardous chemicals such as, hydraulic fluid, thus exposing employees to chemical hazards associated with BRAVO AW hydraulic fluid.
