29 CFR  1910.133(a)(1):  The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a)  On or about October 5, 2020, the employer did not ensure that employees who worked on the catwalk, in direct contact with the open plating tanks, wore safety glasses or a face shield to protect them from eye injuries while exposed to corrosive chemicals (muriatic/hydrochloric acid, nitric acid,  sodium hydroxide) used in plating.  (b) On or about November 19, 2020, the employer did not require all employees to wear safety glasses while hanging parts on production lines where the employees were exposed to mineral and canola oils (eye irritants) that were used to prevent raw parts from rusting before plating.

29 CFR  1910.151(c):  Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  (a)  On or about November 19, 2020 the emergency eyewash and shower station for zinc/iron plating line A was blocked by boxes of raw product waiting to be plated.  (b)  On or about November 19, 2020 the emergency eyewash and shower station for zinc/iron plating line B was blocked by a stack of parts racks leaning against the wall.

When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded. 29 CFR  1910.1200(h)(1):  Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  (a)  The employer did not provide new employees or temporary employees with information and training on the chemicals used in the plating process, such as but not limited to hydrogen peroxide (corrosive), nickel carbonate (potential carcinogen), sodium hydroxide (corrosive), muriatic acid (corrosive), sulfuric acid (corrosive), phosphoric acid (corrosive), nitric acid (corrosive), and mineral oil lubricant (irritant), prior to assigning these employees to hang raw parts on racks beside the plating tanks.

29 CFR  1910.1200(j)(2)(ii): All employers shall, as necessary, update any alternative workplace labeling used under paragraph (f)(6) of this section, update the hazard communication program required by paragraph (h)(1), and provide any additional employee training in accordance with paragraph (h)(3) for newly identified physical or health hazards no later than June 1, 2016.
