29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section: a. On or about November 18, 2020, on the second-story floor decking, three framing employees setting wall framing at the unprotected floor edges were exposed to a 9 feet and 6 inches high fall hazard in that they were not protected by a fall protection system. b. On or about November 18, 2020, in the roof truss webbing, a framing employee installing truss bracing on the bottom cord of the roof trusses was exposed to an 8 feet high fall hazard in that he was not protected by a fall protection system. c. On or about November 18, 2020, on the top cords of the roof trusses, a framing employee installing truss bracing was exposed to an 18 feet and 6 inches fall hazard in that the rope grab on his personal fall arrest equipment was positioned with excessive slack that would have allowed him to hit the ground in the event of a fall.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a. On or about November 18, 2020, the employer had not trained employees to recognized fall hazards while using personal fall arrest equipment in that a framing employee did not know how to select an anchor device and did not know to adjust lanyard length. b. On or about November 18, 2020, the employer had not trained employees to recognize fall hazards from working at unprotected edges of flat working surfaces.
