29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about November 24, 2020, at a site located on 1959 Spring Stubener Rd. in Spring, TX, employees were exposed to a fall hazard while using an extension ladder that was not extended at least 3 feet above the upper landing surface.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about November 24, 2020, at a site located on 1959 Spring Stubener Rd. in Spring, TX, employees were exposed to a fall hazard of more than 6 feet while working on a roof installing a canopy without a fall protection system.  The Ezzi Signs Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1210828, citation number 1, item number 2 and was affirmed as a final order on 05/03/2017, with respect to a workplace located at 8415 FM 1960 W Houston TX 77070.
