29 CFR  1926.451(c)(2)(iv):Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use.  At this work place, employees conducted construction operations greater than six feet above the lower level, and worked from a scaffold platform elevated by an all terrain forklift truck not designed by the manufacture for such use.

29 CFR  1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.   At this work place, employees conducted construction operations greater than 6 feet above the lower level, and did not have a fall protection system in place.
