29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) On or about November 3, 2023, at the jobsite, where employees were exposed to struck-by hazards when using a pneumatic nail gun to install wood framing pieces without safety glasses.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  a) At the construction site, on or about November 3, 2023, a JLG SkyTrak forklift (Model: 8042 T4F) was used to support a scaffold platform that was not designed by the manufacturer, exposing subcontract employees to fall hazards.  Lone Star Building Solutions, LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.451(c)(2)(iv), which was contained in OSHA inspection number 1505303, citation number 1, item number 1 and was affirmed as a final order on August 2, 2021, with respect to a workplace located at 3010 Hwy 181, Portland, TX 78374.
