29 CFR 1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about December 22, 2020, the employer failed to require the use of a protective helmet by an employee installing water lines in an excavation who was exposed to struck by hazards when excavation equipment was operated above the trench, and items such as, but not limited to, a pallet, were lowered into the trench above the employee.
