29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   (a) At the worksite:  On or about 1/15/2021, employees were not protected from fall hazards while accessing a roof surface from a Skyjack Scissor lift,  approximately 30 feet above the lower level, with the lift positioned approximately 2 feet from the edge of the roof.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   (a) At the worksite:  On or about 1/15/2021, employees engaged in solar panel activities on a low sloped roof (approximately 2/12 pitch), were not protected by a guardrail system, a safety net system or a personal fall arrest system, exposing the employees to a fall hazard of approximately 30 feet.
