29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On 1/15/2021, employees of Kuesel Excavating Co. were working inside a trench located at 1500 Highway A, Wentzville, Missouri. The employer did not provide or ensure that a safe means of egress was located in the excavation. The trench used by employees measured approximately 60 feet in length, 9 feet wide and 9 feet deep.

On 1/15/2021, employees of Kuesel Excavating Co. were working inside a trench located at 1500 Highway A, Wentzville, Missouri. The path to enter and exit the trench measured approximately 60 feet long, 6 feet deep and 9 feet wide, workers navigating the path were exposed to the hazards of cave-ins.
