29 CFR 1926.651(b)(3):When excavation operations approach the estimated location of underground installations, the exact location of the installations shall be determined by safe and acceptable means. On or about January 21, 2021, the employer did not ensure that the exact location of underground electric lines were determined by safe and acceptable means. The contractor struck the underground transmission lines, operating at approximately 7,200 VAC each, with the bucket of his excavator during the process of removing earth to install water lines for a building expansion.
