29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.   a) Forth Level Deck: A subcontractor engaged in framing activities without any means of fall protection, sustained substantial injuries after falling through an unprotected shaft approximately 20 feet onto a concrete slab, on or about 1/19/21  b) Third Level Deck - North: A shaft cover was missing, exposing employees and other workers to fall hazards of approximately 10 feet onto a concrete slab, on or about 1/20/21.  c) b) Third Level Deck - South: A shaft cover was missing, exposing employees and other workers to fall hazards of approximately 10 feet onto a concrete slab, on or about 1/20/21.

29 CFR  1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  a) Forth Level Deck: A subcontractor sustained substantial injuries after falling through an unprotected shaft approximately 20 feet onto a concrete slab, on or about 1/19/21  b) Third Level Deck - North: A shaft cover was missing exposing employees and other workers to a fall hazard of approximately 10 feet onto a concrete slab, on or about 1/20/21.  c) b) Third Level Deck - South: A shaft cover was missing exposing employees and other workers to a fall hazard of approximately 10 feet onto a concrete slab, on or about 1/20/21.

29 CFR  1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.   a) Third Level Deck - North: A stud was missing on a shaft, exposing employees and other workers to a fall hazard of approximately 10 feet onto a concrete slab, on or about 1/20/21.  b) Third Level Deck - South: A stud was missing on a shaft, exposing employees and other workers to a fall hazard of approximately 10 feet onto a concrete slab, on or about 1/20/21.
