(a)  Reynold's Construction job #42004 in Parker, CO: On or about February 2, 2021, and at times prior, the employer exposed employees to struck-by hazards from falling or sloughing soil while performing 30" sewer line installation operations in an excavation at 17.9 feet in depth in Type C soil. The employer had not provided an adequate protective system prior to employee entry.  The employees were working within an 8 foot deep trench box without end shielding and with some benching present.  The employer did not comply with the provisions of 29 CFR 1926.652(b)(1)(i), in that the walls of the trench were sloped at an angle steeper than the maximum allowable angle of one and one-half horizontal to one vertical (34 degrees).  Reynolds Construction, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1486735,  citation number 1 item number 1, and was affirmed as a final order on 09/10/2020, with respect to a workplace located at 3555 Grant St. in Denver, CO 80203.
