Located at 541 Hummingbird Court, Lithopolis, Ohio: On or about February 3, 2021, the employer did not ensure that employees operating pneumatic framing nailers and a circular saw were provided with eye protection to protect from flying wood particles and nails.

29 CFR  1926.20(b)(1):It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a. Located at 541 Hummingbird Court, Lithopolis, Ohio: On or about February 3, 2021, the employer failed to maintain an effective safety and health program onsite to ensure that hazards were identified and corrected onsite while performing framing work.  Guarquila Construction LLC was previously cited for a serious violation of this Occupational Safety and Health standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 1477312, citation number 1, item number 1 and was affirmed as a final order on July 23, 2020, with respect to a workplace located at 6964 Hanby's Loop, Lot 42, New Albany, Ohio 43054.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   a. Located at 541 Hummingbird Court, Lithopolis, Ohio: On or about February 3, 2021, the employer failed to ensure employees were protected from falls while performing framing work from the roof, thereby exposing employees to fall hazards from approximately twenty foot.   Guarquila Construction LLC was previously cited for a serious violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1477312, citation number 1, item number 2 and was affirmed as a final order on July 23, 2020, with respect to a workplace located at 6964 Hanby's Loop, Lot 42, New Albany, Ohio 43054.
