29 CFR  OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crush hazards and struck-by hazards during the operation of powered industrial trucks without the use of restraint systems (seatbelts):   (a) On February 5, 2021; an employee operated the CAT GP25, S/N GP25SAM91562 powered industrial truck without the use of a seatbelt.   (b) On February 5, 2021; an employee operated a Toyota, S/N 8fgu2519592 powered industrial truck without the use of a seatbelt.   Note - Abatement certification is required for this item.

The operator was traveling forward and his view was obstructed by the load.     (b) On February 5, 2021; an employee operated a Toyota, S/N 8fgu2519592 powered industrial truck to move multiple bins of onions between the staging area outside of the Packing Line and the empty bin staging area. The operator was traveling forward and his view was obstructed by the load.   Note - Abatement certification and documentation are required for this item.
