29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  On or about February 22, 2021, at 20830 Kuykendahl Rd. in Spring,  TX, employees were exposed to a fall hazard of more than 6 feet while performing roofing activities on a roof of a commercial establishment without the use of any fall protection system.   Hugo Estrada was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1305615, citation number 1, item number 2 and was affirmed as a final order on 10-19-2018, with respect to a workplace located at 3000 South Grand Ave. Springfield, IL 62703.  Hugo Estrada was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1199478, citation number 1, item number 1 and was affirmed as a final order on 03-16-2017, with respect to a workplace located at 1804 K Avenue. Plano, TX 75074.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about February 22, 2021, at 20830 Kuykendahl Road in Spring, TX, employees were exposed to a fall hazard when using an extension ladder that was not extended at least 3 feet above the upper landing surface.  Hugo Estrada was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1305615, citation number 1, item number 1(b) and was affirmed as a final order on 10-19-2018, with respect to a workplace located at 3000 South Grand Ave. Springfield, IL 62703.

29 CFR  1926.95(a):Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided.  On or about February 22, 2021, at 20830 Kuykendahl Road in Spring, TX, the employer did not ensure that employees handling aluminum sheets, wood, using hammers, nail guns, knives, screws, nails, and abrasive materials, used an appropriate well-fitting work glove that is resistant to punctures and cuts.
