(a)  Facility:  On or  about March 10, 2021, a maintenance employee was working on the K Plant Sacker machine and it was not electrically locked out.  In the lockout/tagout energy control procedure for that K Plant Sacker, (that was updated on 3/9/21) step 3 reads as follows:  3.  In the circuit breaker panel beside the bulletin board, locate Breaker #26 and turn it off.  Apply a Circuit Breaker Lock to this breaker and attach a lock and tag.  This Breaker #26 did not supply any electrical power to the K Plant Sacker.
