29 CFR  1910.147(d)(3):  All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  (a)  Facility:  On or about January 4, 2021, the employer did not ensure that the #1 K Mixer was locked/tagged out before an employee removed a panel (guard) to the machine and attempted to unjam the machine with a steel bar.   The Resco Products Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1520415, citation number 1, item number 1D and was affirmed as a final order on 7/2/2021, with respect to a workplace located at West 6th and Center Street, Tarentum, PA 15084.

29 CFR  1910.147(c)(7)(iii)(A):Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures:   (a)  Facility:  On or about December 14, 2021, employees needed to be retrained in how to review a lockout tagout procedure and in how to ensure that a machine is locked/tagged out before work is begun on the machine.  Every employee at the facility is authorized to lock out a piece of equipment but the employer only trained 5 employees from the previous inspection (#1520415).  (b)  Facility:  On or about January 3, 2022, employees need to be retrained in how and when to lockout/tagout a piece of equipment.  Employees were working to clear a jam in the #1 K (Erlich) Mixer with a guard removed and the machine was not locked out.   The employees were trying to use an approximately 6 foot long steel bar to clear a jam.   The Resco Products Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29  CFR 1910.147(c)(7)(iii)(A), which was contained in OSHA inspection number 1520415, citation number 1, item number 1b and was affirmed as a final order on 7/2/21, with respect to a workplace located at 6th and Center Streets, Tarentum, PA 15084.

29 CFR  1910.22(a)(1):29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition:  (a)  Facility, Mill Room and Doghouse at Silos:  On or about December 15, 2021, the Mill Room and the top and bottom areas around the silos needed to be cleaned from a buildup of refractory dust.    The Resco Products Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(1), which was contained in OSHA inspection number 1520415, citation number 2, item number 2 and was affirmed as a final order on 7/2/21, with respect to a workplace located at West 6th and Center Street, Tarentum, PA 15084.
