29 CFR  1910.28(b)(15): Walking-working surfaces not otherwise addressed.  Except as provided elsewhere in this section or by other subparts of this part, the employer did not ensure each employee on a walking-working surface 4 feet (1.2 m) or more above a lower level is protected from falling by one of the systems listed in 1910.28(b)(15)(i)-(iii).    On or about 03/13/2021 and at times prior, at 475 North Rd, Naples FL, the employer did not ensure that an employee servicing a Hoist Lift Truck (i.e. Neptune, M250/2015 SN 30223) at approximately 15 feet or more above a lower level was protected from falling.

1910.147(c)(6)(i)     The employer did not  conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  On or about 03/13/2021 and at times prior, at 475 North Rd, Naples FL, the employer did not inspect its energy control procedures to ensure that the procedure for isolating all sources of energy during the replacement of a hydraulic hose of a Hoist Lift Truck  (i.e. Neptune, M250/2015 SN 2033 )  were followed, thus exposing an employee to a caught by hazard.
