29 CFR  1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  a)  Roof Deck:  On March 30, 2021, Employees engaged in steel erection activities were exposed to fall hazards of approximately 33 feet while puddle welding and cutting metal decking.  NOTE: Abatement certification is required for this item.

29 CFR  1926.451(b)(1)(ii): The scaffold platform was not planked or decked as fully as possible and the remaining open space between the platform and the uprights exceeded 9.5 inches (24.1 cm):  a)  Second floor, northwest corner:  On March 25, 2021, an iron worker was welding bridging from mobile tubular welded end-frame scaffold (aka "Bakers" scaffold).  The worker was on only a single approximate 18" wide plank and the rest of the deck was not complete.
